We are a faith family devoted to helping one another love Jesus, grow in Jesus, and make disciples of Jesus in Birmingham and among all nations.
Our Local Disciple-Making ministries exist to care for our members in biblical community, help them grow in their faith, and equip and mobilize them to make other disciples together.
Our goal is to be a faith family of world-impacting disciples who are partnering with our brothers and sisters here in Birmingham and around the world to produce ever-multiplying churches all around our city, this country, and to the ends of the earth.
We offer all kinds of resources we produce, use, or recommend to equip both our faith family and other churches.
Parents, please help your child complete all of the following fields and submit by clicking the button at the bottom of this page. Responses should be written in your child's own words. Don't concern yourself too much about whether or not his/her language is "theological" enough. You may find it helpful to utilize a piece of paper or a separate word processor to help you formulate his/her answers before populating the fields.
If you and your child will be unable to complete the assignment in one sitting, please save them elsewhere because you WILL NOT be able to save them on this form and come back to complete it. If you wish, you may complete this in a Word Document, or turn in a hard copy available at the Welcome Center.
Read "God's Story and Me" with your child on pages 5-7 in our Membership Guide for Kids available at brookhills.org/allin under ALL IN Membership Resources for Kids. Then use the following prompts to help your child write the gospel in his/her own words.
We all have a story to tell: our story of how God helped us hear, understand, and believe the gospel and how he has changed us and continues to work in our lives. We often call this story our “testimony.”
Now that your child has thought through his/her responses to the questions above, help him/her write out his/her testimony in about 150-200 words.
EXAMPLE: God is the Creator of everything. He is caring, loving, and everything you could hope for in a loving Father. I was visiting my granddad last summer and told him I wanted to be baptized. He and I read some verses together in the New Testament he gave me. We read Romans 3:23, and I understood all have sinned and knew that included me. We talked about how everyone has fallen short of the glory of God. John 14:6 says, “I am the Way, the Truth, and the Life. No one comes to the Father except through me.” Jesus lived a sinless life so he could die and take the penalty for my sins. I knew I needed Jesus. Then we read Romans 6:23, and I understood I needed to turn from my sin by accepting the gift from God of eternal life in Jesus our Lord. I prayed asking Jesus for forgiveness of my sins and for help following the example he gave us. I have a Christ-loving family who makes worshiping him a priority, and several leaders at Brook Hills have helped me engage with the gospel. I want to follow the example Jesus gave by getting baptized. Today I confess in the church and to all the world that Jesus is my Lord and Savior.
If your child has not been baptized and desires to be, have him/her also answer the following question. Then complete and submit his/her testimony using the instructions below. Your child will read his/her testimony aloud when he/she is baptized.
If preparing testimony for baptism, have your child include his/her name and age at the beginning of the testimony. Have him/her include this statement at the end of the testimony. “Today, I want the church and the world to know that Jesus Christ is my Lord and Savior.” This all can be in addition to the 150-200 words.
